1.Przedmiotem zamówienia jest świadczenie usługi transportu osób dializowanych w Stacji Dializ Zamawiającego położonej przy ul. Kamieńskiego 73a. Usługa dotyczy wszystkich pacjentów Stacji Dializ, wymagających transportu, w tym pacjentów chodzących, pacjentów z ograniczoną możliwością poruszania się oraz pacjentów leżących. 2.Przedmiot zamówienia obejmuje świadczenie usługi transportu pacjentów dializowanych w systemie całodobowym, przez wszystkie dni tygodnia, przez cały okres obowiązywania umowy w zakresie transportu osób dializowanych w Stacji Dializ Zamawiającego, położonej przy ul. Kamieńskiego 73a. 3. Zamawiający dopuszcza, aby przedmiot zamówienia obejmujący transport pacjentów do Stacji Dializ, był świadczony środkami transportu sanitarnego i osobowego.
